Section titled “Reporting an incident”If you witness or are involved in a health and safety incident at school — an accident, a near-miss, a property-damage event — you can file it directly without needing to find the H&S lead first.
How to file
Section titled “How to file”- From the portal, open Report an incident (in the left rail, or directly at
/me/report-incident). - Fill in the form:
- Category — accident, near miss, dangerous occurrence, hazardous release, security, fire, environmental, behavioural, or other.
- Occurred at — date and time. Default is now, adjust if it happened earlier.
- Location — free-text. “Year 5 classroom”, “playground near the climbing frame”.
- Description — what happened, in plain words. The form requires at least 20 characters; aim for more.
- Immediate action taken — optional. What you did at the time.
- Submit. You land on a thank-you page.
What happens next
Section titled “What happens next”The H&S lead picks up the row from the agent incidents register. They’ll:
- Assign a reference number.
- Decide whether it’s RIDDOR-reportable.
- Investigate (interview witnesses, examine the scene).
- Close the loop with you if needed.
You don’t get an agent-side surface to follow your incident — if you need an update, raise a separate ticket asking for it.
Severity defaults
Section titled “Severity defaults”The form keeps severity defaulted to low — you don’t have to make that judgement. The H&S lead recalibrates during triage based on the description and any follow-up.
Why the form is narrow
Section titled “Why the form is narrow”Deliberately. The H&S regs say anyone can file; this surface honours that. A more thorough form would deter people, and we’d lose near-miss reports that should be filed (because nothing actually went wrong, the impulse to file fades fast).
If you’re an agent and want to file something more detailed, use the agent incidents register directly.
Gotchas
Section titled “Gotchas”- You can’t edit your own incident after submission. Add detail in a separate ticket if you remember more.
- The system records who filed (you). You can’t file anonymously through this surface.
- If someone is seriously injured call 999 first. Then file the incident afterwards.
