The risk assessments surface is the trust’s central RA library. Each RA covers a hazard, activity, or location — “Working with chemicals in the science lab”, “School trip — coach travel”, “Manual handling of staging blocks”. Once published, an RA can require staff to acknowledge it before participating in the relevant work.

The lifecycle

draft → approved → published → archived
  • Draft — author writing. Visible to other agents, not yet binding.
  • Approved — reviewed and approved by the H&S lead (or someone with the hs::risk_assessments::manage permission). Can be published directly from this state.
  • Published — the live version. Staff are bound by it and may be asked to acknowledge it.
  • Archived — taken out of service. Not surfaced in normal queries but retained for the audit trail.

What’s on an RA

  • Title + category — “School trip — coach travel” under category Trips.
  • Hazards identified — list of risks (slips, fire, allergen exposure).
  • Existing controls — what’s already in place.
  • Risk scoring — likelihood × severity, before and after controls (residual score).
  • Residual score — the headline number after controls (likelihood × impact). Colour bands: red ≥ 15 (High), amber 8-14 (Medium), yellow 4-7 (Low), grey below 4 (Negligible).
  • Additional controls required — what else needs doing.
  • Next review date — when this needs revisiting. Drives the dashboard tile.
  • Requires acknowledgement — flag. If set, every staff member doing the relevant work has to acknowledge before participating.

Versioning

RAs are versioned, not edited in-place. When you change a published RA, the new version goes into draft until reviewed and published. The previous version stays in the audit trail.

The portal-side RA browse shows only the latest published version.

Gotchas

  • The residual score is what matters for the traffic-light. A high inherent score with strong controls is fine; the system tracks both but only the residual drives the colour band.
  • Requiring acknowledgement and then archiving does not remove pending acknowledgements — staff will still see the row on /me/risk-assessments until they acknowledge or until the RA is archived and they refresh.
